1. Contact our office to complete an intake (This usually takes 10-15 minutes). During this intake, we will get information from you to help us determine what help is needed and how we can serve you best.

2. Contact your physician to fax us a prescription

3. Check your health insurance regarding your speech- and/or occupational- therapy benefits. Ask them about coverage, limits, and patient responsibility amounts.
4. Complete the appropriate form below prior to the first visit (You may email it to us prior to your appointment or bring it with you to your appointment)
