1. Contact our office to complete an intake (This usually takes 10-15 minutes). During this intake, we will get information from you to help us determine what help is needed and how we can serve you best.

2. Contact your physician to fax us a prescription for ST and/or OT services

3. Check your health insurance regarding your speech and/or occupational therapy benefits. Ask them about coverage, limits, and patient responsibility amounts.
4. Complete the form(s) below and bring the paperwork to the first visit
